            With this cover letter is a copy of the 4-H Awards Application for the 2004-2005 4-H Club year that we just finished.

            The forms are the same as last year. You need to send one award form for each award you are applying for. The forms can be copied or call the office for additional copies.

            You may apply for awards in the following areas:
