The North Dakota Rural Health Outreach Grant Program supports projects that demonstrate innovative models of rural health and health care services (including health promotion/disease prevention). The program requires a network or consortium of three or more separate entities be formed to develop and implement the outreach project. Grants are awarded either for the direct provision of health services to rural populations, especially those not currently receiving them or to enhance existing services. There have been 15 separate Outreach grants funded in North Dakota since 1991. The website has a summary of those projects for your review as well as guidelines for developing a proposal.
