Local Initiatives Funding Partners Program is a national
program office of The Robert Wood Johnson Foundation (RWJF).
Program goals include (1) access to health care; (2) improving
quality of care for chronic health conditions; (3) promoting
healthy communities and lifestyles; (3) reducing harm caused
by substance abuse. Interest is for programs that address
childhood obesity, racial and ethnic disparities in
health/health care, and services to vulnerable populations.

The North Dakota Rural Health Outreach Grant Program supports
projects that demonstrate innovative models of rural health
and health care services (including health promotion/disease
prevention). The program requires a network or consortium of
three or more separate entities be formed to develop and
implement the outreach project. Grants are awarded either for
the direct provision of health services to rural
populations,especially those not currently receiving them or
to enhance existing services. There have been 15 separate
Outreach grants funded in North Dakota since 1991. The website
